The CMS GUIDE model
GUIDE, Guiding an Improved Dementia Experience, is a Centers for Medicare and Medicaid Services model for delivering coordinated dementia care. Participating practices take on structured responsibilities around assessment, care planning, caregiver support and ongoing follow-up.
Meeting those responsibilities consistently, for every patient, is an operational problem before it is a clinical one. Intraneuron implements and operates the software workflow that makes it repeatable.
What the platform handles
- Registration and intake. Patients complete registration and intake forms before they arrive, so the visit starts with the paperwork already done.
- Medical and neuropsychological scales. The instruments a practice already relies on, administered and scored consistently rather than re-keyed from paper.
- Audio questionnaires. Scales administered by voice, which is how we measure verbal memory and fluency and what makes remote administration practical for older patients.
- Caregiver-reported measures. Collected as part of the same process rather than chased separately.
- Structured follow-up. Workflow that tracks where each patient is, so scheduled reassessment happens instead of depending on someone remembering.
- Embedded clinical trial recruitment. Identifying candidates for research out of routine care, rather than running recruitment as a separate effort.
These are components rather than a fixed product. Practices differ, and so do the pathways they need, so we build the workflow a practice actually runs rather than asking it to adopt ours.

Memory clinic workflow
We also provide the workflow infrastructure behind memory clinics: intake, structured assessment, scoring, reporting and scheduled follow-up, organised so a clinic runs the same pathway for every patient instead of assembling one per visit.
It is the same platform underneath. A practice running a memory clinic and a practice participating in GUIDE are doing much the same operational work, and both are better served by software built for the pathway than by a general-purpose record system.

Who builds it
Intraneuron was founded in 2013 by Gregory Sahagian, M.D., a neurologist and chief executive of The Neurology Center of Southern California, and Vidar Vignisson, a technology executive with more than thirty years building software.
The platform is developed alongside practising clinicians rather than at a distance from them, including two board-certified clinical neuropsychologists. You can read more about the team.
